  • We accept Doral, Med Plus, Gateway, and UPMC
  • Any patient that does not show up or give 48 hours notice of cancellation for a scheduled appointment will not be seen or rescheduled in this office until a $20.00 fee has been paid.
  • Patients are required to provide proof of insurance (ID Card with group agreement numbers) , so that we can submit the claim information for payment.
  • If you do not have your insurance information, you will be required to pay for the visit before being seen.

  • All major dental work such as Crowns, Bridges, Partials, and Root Canals, must be pre-authorized before your insurance company allows us to start treatment.
  • West Penn Dental Center, Inc. is authorized to use and disclose health/Dental information for treatment, payment, and health/dental care operation purposes consistent with its notice of privacy practices in accordance with our HIPA policy.
