INSURANCE PATIENT

We participate and submit claims for most dental insurance plans for you. We Cannot Guarantee if or what your insurance carrier will pay.

Patients are required to provide proof of insurance ( ID CARD WITH GROUP AND AGREEMENT NUMBERS) so that we can submit the claim information with payment.

If you do not have your insurance information, you may be required to pay for the visit before leaving the office. When you provide the information, we will complete and submit the claim for you so that your insurance company will reimburse you for payment.

Please Note: Blue Cross/Blue Shield Medical and Blue Cross/Blue Shield 65 Special and Medicare do not pay for dental procedures.

Most Dental Insurance Plans do not cover 100% of dental treatment, any co-payment deductibles, or percentage of payment not covered by your insurance company. It is your responsibility to pay for the services rendered at the office.
Dentures, Crowns, Root Canals, and Bridge Work

Due to lab expenses, work on dentures, partials, crowns, and bridgework will not be started until an initial deposit is made.

No Refunds once lab work is initiated. Charges for lab work completed will apply.

Half of the total fee for the procedure is due the day the treatment starts.

The remaining half is due in full on or before the day the treatment is completed or, the appliance is inserted.

Dentures, Crowns, Partials, and Bridgework will not be inserted unless the final payment has been received.

Please Note: Special Financial Arrangements can be made with the office manager to complete your payment on your scheduled appointments. Appointments can be spaced, so that a comfortable payment schedule can be met and payments are completed by the final appointment.
You insurance company benefits booklet will explain what portion of treatment is your responsibility and what portion the insurance company will pay. Please bring in your benefits booklet if you are not clear on what your benefits are. This will also enable us to give you a more precise estimate as to what your payment will be.

X-Rays are usually required by your treating doctor and your insurance company for treatment authorization and consideration for payment.

Any additional costs/fees for legal collection of services incurred by us to collect on an overdue account will be added to your outstanding balance.

A $25.00 fee will be charged for NSF checks returned to us from our bank.

Depending on circumstances, there may be a charge for appointments cancelled or missed with less than 24 hours notice prior to your appointment time.
