We accept Doral, Med Plus, Gateway, and UPMC
Any patient that does not show up or give 48 hours notice of cancellation for a scheduled appointment will not be seen or rescheduled in this office until a $20.00 fee has been paid.
Patients are required to provide proof of insurance (ID Card with group agreement numbers) , so that we can submit the claim information for payment.
If you do not have your insurance information, you will be required to pay for the visit before being seen.
All major dental work such as Crowns, Bridges, Partials, and Root Canals, must be pre-authorized before your insurance company allows us to start treatment.
West Penn Dental Center, Inc. is authorized to use and disclose health/Dental information for treatment, payment, and health/dental care operation purposes consistent with its notice of privacy practices in accordance with our HIPA policy.
